Cardiac Catheterization Lab Nurse
Demonstrates sufficient knowledge of medications to pass cardiovascular special procedure (CVSP) pharmacology questionnaire. Familiar with intra-aortic balloon pump operation, and all supplies and equipment used during standards and specialized procedures. Position Summary: Cardiovascular Special Procedures: In addition to duties and responsibilities of Staff Nurse II position description, electrophysiology and cardiac implants. Assists cardiologist and radiologist with diagnostic and therapeutic cardiovascular procedures. RN should be able to function and circulate in the following cases: left heart cath, right and left heart cath, coronary interventions, Stent placement, IABP, IVUS, rotoblator, radi-wire, EP/A-flutter/A-fib Ablations, Pacemaker/AICD/Bi-V implants and all cases in IR/CT/US. Competencies should include administering medications used in CVSP including sedatives, vasoactive, intra-coronary, intravenous, intramuscular and oral medications including its antidotes. Position Duties and Responsibilities (an asterisk (*) denotes an "Essential Job Function” for ADA purposes) 1.* Performs all nursing aspects relating to cardiovascular special procedures, including patient and family education, and complies with all JCAHO and DHS regulations. Provides oversight and direction for the clinical care of the patient through all aspects of the procedure, circulates and assists during cardiovascular procedures under the direct supervision of the operating cardiologist or radiologist. Additional skills not required at time of hire include scrub and recorder duties. a) Scrub: Verifies peripheral pulses, shaves and prepares patients for procedure, drapes patient and maintains sterile fields; assists physician with catheter and line insertions; injects coronary arteries, pulls arterial and venous lines and achieves hemostasis. b) Circulator: Attaches EKG leads to patient, maintains or starts intravenous (IV) fluids, informs and reassures patient, administers medications and monitors its effects as ordered by attending cardiologist/radiologist. Acquires and opens catheters, wires and supplies as needed by scrub team, and operates high pressure contrast media injector (Medrad). Monitors cath lab during procedures to eliminate unnecessary visitors and foot traffic. Notifies attending physicians with sufficient warning when preparing for procedures, ensuring all necessary supplies, equipment, and medication needs for procedure are immediately available before beginning of procedure. Continually monitors vital signs during the procedures and informs physician of relevant changes.c) Recorder: Prepares all paperwork relating to procedure and maintains a chronological log of procedures and reviews and co-signs CV Tech procedure documentation. Records and monitors EKG and hemodynamic data informing physician of abnormal rhythms. Continuously monitors vital signs during the procedures and informs physicians of relevant changes. Traces ventricular outlines and obtains ejection fractions, calculates cardiac outputs in valve area by computer.
