Case Management Nurse - Case Manager

The Staff Nurse (SN) II is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame. The SN II provides care to patients via the therapeutic use of self, the nursing process, the environment and instrumentation, and other health care team members. One year recent (within the last 3 years) experience in the field of specialization such as asthma, cardiovascular risk conditions [e.g., diabetes, pediatric diabetes, COPD, cholesterol, heart failure, hypertension], care of the frail elderly, pain management, chronic kidney disease/kidney failure, other chronic conditions. 8 hr shifts 40 hours per week w/ vaired days per week as needed.

Required Qualifications

BLS ** Case Manager needed w/ Experience** Skilled Nursing Facility Experience 3 year of experience with acute care, inpatient case management Effective written and oral communication skills CA license and California experience as a case manager is a must Evaluate home Anticipate needs, DME Troubleshoot special circumstances ie difficult placement. Comfortable with technology, EMR, charting in EMR.
